The Nigerian Health Sector; Just Before The Waterloo

Judging by the facts of the issues that has led  to the present action of the NMA, it is clear to me that if urgent and  decisive steps are not taken then the Nigerian Health sector is just  about to meet its waterloo.<a name='more'></a><br /><br />No doubt NMA and her doctors have received the worst bashing from the  general public; which has been fueled by the support of the Union of the  Joint health workers under the aegis of an amorphous body called  JOHESU. This situation is so because people lack dept into the issues  that has caused the present action of the doctors and of course the fact  that they cannot phantom why the doctors would appear so callous as to  abandon their patients.<br /><br />If we must avoid impending doom it is imperative that we take a critical  impartial analysis of the present issues, so that we can take decisive  steps to avoid the worst coming to being...<br /><a href="https://www.blogger.com/blogger.g?blogID=1273158107245326844" name="more"></a><br />Rather than just always criticize the doctors for withdrawing services,  it is better to first understand the issues dispassionately; next we can  then query the decision of the doctors to have taken withdrawal of  service as the option to pursue their agitation.<br /><br />For all those who have good knowledge of the goings-on in the health  sector of Nigeria, it would be clear that whatever crisis we are  witnessing now is a result of conflict that has long bedeviled the  sector and has been brewing for over two decades ago.<br /><br />Without prejudice to any of the disputing sides, i make bold to say that  health service requires the collaborative efforts of various  professionals and other allied workers whose only goal is to come to a  positive outcome of patient care. Every part of this whole has its  integral role that it plays and one part may not claim superiority over  the others.<br /><br />However, in any scenario where we have different people coming together  to achieve a single goal, there must be direction and this direction  must be provided by leadership. The problem in the Nigerian health  sector is that leadership has been misconstrued as superiority and as  noted earlier, this problem has been on for decades. I also make bold to  say that both sides are guilty of this misnomer.<br /><br />Much as we try to have a clear understanding of the distinction between  leadership and superiority, we must however also have a clear mind as to  who to Captain the ship. It is commonsensical that only one captain is  needed to direct this Ship, and anything other than that leads to chaos;  which is exactly what we must avoid.<br /><br />It is also commonsensical that the best person to captain the ship is  the most knowledgeable person about the entire process and without doubt  that positions falls to the doctor. That the doctor is the one with the  most knowledge of the patient is what the other health professionals  find discordant to their ears and this has been the bane of the long  standing conflict.  But no one gets annoyed or argue when the lawyer  says that he is the only learned person. Why should this concept be a  problem within the health sector.<br /><br />The common man may however find it petty that such issues should be  taken so seriously by the doctors and more so resulting in such serious  action as withdrawal of services. I must however paint the full  implication of the issues as it affects the medical profession and  ultimately how it affects the care of the patients too.<br /><br />What the other health workers are asking for is not just a name, but to  use and be accorded what is due to the term consultant. Other than that  they also believe that it is their right to also head the health team.  (Using the term health team gives a clearer implication of this  request.)<br /><br />Let us all be clear that the term consultant may refer simply to an  advisory role between a professional and its client. Within the hospital  setting however this is not so. The term consultant refers to a  specific role which describes a relationship between a patient and a  physician who has <div data-cid="2378" data-kw="completed" style="border-bottom: 1px solid; color: #f35b00; cursor: pointer; display: inline-block; height: auto; padding-bottom: 1px; text-decoration: underline; width: auto;">completed</div>his specialization in a particular part of medicine. It is a position of responsibility as well as authority.<br /><br />The implication is that if a doctor does not become a specialist,  irrespective of many years of practice or getting to the highest  position in service, it still does not confer on him the title  consultant. What do we then make of a Nurse or pharmacist who wants to  attain the same status. It has been believed that because these other  health workers now pursue postgraduate degrees up to attaining a PhD it  is therefore expected that it is okay for them to receive such status.  But pursuing an academic career is completely different from  specialization. Even a medical doctor who decided to attain a PhD does  not get the title consultant.<br /><br />The implication of having other health workers attain the status and  role of consultant is that it provides more than one captain to direct  the ship. For instance the consultant pharmacist may believe that he is a  specialist in the field of drugs and therefore may decide on what drug a  patient may or may not be given. Everyone else including nurses and  laboratory scientist may decide on themselves what they want or do not  want for the patient. There exists no other word for this other than  chaos!!<br /><br />How would a patient get well with conflicting orders? Indeed who would  be ready to get on board a ship with more than one captain. Well I  wouldn’t, even if the ride is for free. Let it be clear that the  scenarios painted above are not just suppositions, they are realities  that already exist even before the conferment of this title. The doctors  have however been able to save the situations in the past by referring  the other team members to their specific roles and responsibilities. Now  that the title has been conferred on other health workers it has been  impossible to get proper direction for patients’ care in places where  this happened and patients have been at the receiving end of it all.<br /><br />With respect to the issue of headship, a clear distinction must be made  between the head of administration of a hospital and the Chief Operating  Officer. The position of the head of administration of the hospital has  been occupied by people other than medical doctors. The NMA has no  issue with anyone with the requisite skills to occupy that position. But  when it comes to the chief operation officer of the hospital; by  whatever name it is called, Medical Director, CMD, CEO of CFO, that  position we maintain should be headed by a Medical doctor.<br /><br />This is a position of high responsibility and one in which the officer  has to make judgment call that is not based on just administrative  matters but that is based on the clinical responsibility of the  hospital. We must have at the back of us our minds that the goal of the  hospital is patient centered. It is ONLY (note emphasis) the doctor that  has the COMPLETE knowledge of the patient and can take most  responsibility regarding patient care. It is therefore a desideratum  that the doctor only can occupy this position in other to deliver on the  expected goal of the hospital and health systems.<br /><br />One cannot but be puzzled and amazed at the rate at which other health  workers have jostled for the top post. These are people who are indeed  very protective of their position and roles. Would the pharmacist allow  the pharmacy department to he headed by a pharmacy technician? Would the  laboratory scientist prefer to take instruction form the technicians?  We know the answer to these questions. I also know as a matter of fact  that there is a law in this country that bars non radiographers from  using radiological equipments even the technician within that department  is not allowed.<br /><br />It is dismal that while these group of health officials are highly   engaged in professional territorial protection, they are also agitating  for the job, authority and responsibility of the doctor. This and the  chaos that may arise from it is what the doctors are protecting for  engaging in the current industrial action.<br /><br />Haven understood the basis of the industrial disharmony one must still  ponder why the option of withdrawal of services has been taken by the  doctors; especially when the grave consequences are well known.<br /><br />In any reasonable society doctors are not to be allowed to go on strike  under whatever condition. The onus however is not left to the doctors  alone but also to the government and the general public. We must  remember that the doctors are also a part of the society and become  subjected to the same factors affecting everyone. The physician oath  indeed recognizes this fact and provides that a physician should be well  treated by the society as long as he plays the role of dedicating his  time to patients care.<br /><br />While one cannot take away the option of the doctors to agitating for  their well being amongst other things, it is a terrible thing for  doctors to withdraw services especially in a large scale. I believe that  other methods must be available for the NMA to pursue its demands. We  know that dialogue do fail but even when pushed to the wall the option  of strike is very grave.<br /><br />The organized labor NLC, TUC, etc and civil society groups should rather  come to the aid of the doctors and serve as pressure groups to pursue  the clear demands and aspirations of the doctors. But what would you  have NMA do when those who we believe are supposed to be the  intercessors are the ones who actually worked against the NMA.<br /><br />As a matter of fact, while JOHESU had gone on strike a couple of times  over this issue in the last one year, NMA has averted the same course by  postponing an industrial action at least four times last year alone.  NMA had continued to embrace the part of dialogue to pursue its demands.  The game changer was the point when the Nigerian Labour Congress teamed  up with the other health workers and threatened the federal government  to sign the agreement with JOHESU even without prior fair hearing from  NMA.<br /><br />With the Federal government haven signed an agreement with JOHESU which  is the complete reverse of the understanding with the doctors, NMA is  left with no other choice than to take her destiny into her own hands.  This is the reason for the current action of NMA and I sincerely await a  dispassionate suggestion as to what other methods NMA could have  adopted. The ultimatum given to the Federal government was a public  document which was put on the pages of the news paper, Nigerians;  including the civil society group did not deem it wise to intercede  during that time frame and we all allowed the NMA to proceed on the  withdrawal of services.<br /><br />We all have a blame in this. The NLC, TUC and civil society groups have  failed the country. We do not just claim to be a sane society without  living up to such responsibilities. Up till today members of the civil  society groups have not deem it fit to come into this present imbroglio.<br /><br />As noted earlier, worse times are coming. How do we expect to achieve  the team spirit with so much passion and acrimony. This is only a  forbearer to unending disharmony. There is no team that can deliver  without harmony, and there is no way the health sector can deliver on  its goals without a team spirit. We must not wait to get to the end of  the line. We must all act fast and decisively to forestall this grave  consequence.<br /><br />In other to move forward, the government must first find a way for the  NMA to suspend its industrial action. One way is to ask that the whole  team return to status quo ante. It is clear that only in settled waters  do things mirror themselves, undistorted. Only in settled waters can we  find a lasting solution to the problem at hand. No half measures.<br /><br />The NMA as the rightful leader of the health sector must understand what  leadership truly confers. It is a responsibility not only to the  doctors but to everyone within the health sector. This is a role NMA has  not truly delivered over the years. Only when the NMA assume this  rightful responsibility can the other members of the team be ready to  follow her leadership.<br /><br />NMA and other health workers must come to a round table with a no winner  no vanquish spirit. Together they can plot a future for the health  system of this country. A future that cater for the needs and  responsibilities of every member of the team, a future that is patient  centered, geared towards improving quality in health care and elevation  of the various professionals and allied groups within the health  workforce.<br /><br />This is the only dream that is truly positive. The only vision that can  be pursued and lead to a positive future. Lets all swallow our pride and  act wisely. The only  incontrovertible sponsors of Boko Haram today are the nation’s army  under your minister of defense, office of <span class="q81g0lm7" id="q81g0lm7_4"><div data-cid="2378" data-kw="national security" style="border-bottom: 1px solid; color: #f35b00; cursor: pointer; display: inline-block; height: auto; padding-bottom: 1px; text-decoration: underline; width: auto;">National Security</div></span>, oNSA, chief’s of the army and your <span class="q81g0lm7" id="q81g0lm7_2">executive office</span>,  who give Boko Haram the nation’s weapons, tanks, APC’s and $20 billion  in fund reserves and provide them helicopter and air force jet cover to  drive freely in 40 car convoy’s devastate local farming communities of  the north for hours on end, displacing the third largest number in  recent recorded history (3+million) while never being able to check  their reign of limitless kidnapping, incendiarism, and extermination. <br /><br />Do you think that any other civilized or uncivilized nation on earth  other than Nigeria the world BS capital you have made it, would tolerate  this senseless excuse more than once with no follow-up <span class="q81g0lm7" id="q81g0lm7_3">security service</span> arrest of the’ implicated’ individuals… talk less tolerate the rubbish  for five full years? If it is Buhari, arrest him, if it is Tinubu,  arrest him. Do you think we do not know that when Buhari dragged your  behind to court, your spokesperson, Reuben Abati who first manufactured  and launched this puerile libelous, lame excuse, was forced to apologize  and together with Guardian where his demonic concoction was published,  published an apology to Buhari’s person in the Guardian of Thursday 11th  July, 2013, captioned ‘Re: For the attention of General Buhari?’ <br /><br />You are lucky the opposition ‘officials’ in Nigeria are nearly as hopeless as you, else you would have been in local and <span class="q81g0lm7" id="q81g0lm7_5">international court</span> along with all your friends who keep terrorizing Nigeria with this  garbage every day for the past five years.  Arrest them or forever zip  it. And if you want to know people, terrorist candidates with clear  evidence who should be arrested already, look beside you at Abba Moro,  the self confessed agent of Satan, who terrorized millions of your  youths and killed two dozen. Why is he still walking free rather than in  jail, and worse yet, still at the head of our ministry? These obvious and painful  self-evident truths gave them goose pimples. At every angle the  organization was dazed with the weighty and heavy credentials of Chief  MKO Abiola. In every issue, Tofa could not get it and had no answer to  any question thrown to him. Devastation, embarrassment and confusion hit  the organization and there was no solution in sight. Suddenly an idea  came to the campaign organization.<br /><br /><b> What did the campaign organization headed by Dr Doyin Okupe do? </b><br /><blockquote class="tr_bq">They told themselves that since they cannot match Abiola’s moving train  on capacity, experience, boldness, wealth, education, character,  charisma, local, national and international connections, intelligence,  common sense etc the <span class="q81g0lm7" id="q81g0lm7_8">best thing</span> to do is to try as much as possible to pull down Chief MKO Abiola by  any means possible from his towering height to Tofa’s Lilliputian level.<a name='more'></a></blockquote><div><b>But did they succeed?</b></div><div><a href="https://www.blogger.com/blogger.g?blogID=1273158107245326844" name="more"></a></div><div>They never did. It was an <span class="q81g0lm7" id="q81g0lm7_12">exercise</span> in futility, all noise and no substance, a tale told by some nitwits full of <span class="q81g0lm7" id="q81g0lm7_2">sound and fury</span>, signifying nothing. Chief MKO Abiola won a land slide victory.<br /><br />22 years after, history is repeating itself in Nigeria. The All  Progressives Congress, APC was formed last year to challenge the ruling  Party PDP whose leaders had claimed that it will rule Nigeria for  60years. Realizing that if nothing tangible is done to stop Nigeria from  becoming a one party state and consequently turning into a <span class="q81g0lm7" id="q81g0lm7_10">full blown</span> dictatorship, the ANPP, ACN, CPC and a faction of APGA came together to  form APC after an excruciating and daunting exercise. Nigerians are  living witnesses of how these legacy parties walked through the mines,  drilled the deepest well, bite bullets, and faced many battles with  bruises and pains to achieve this feat. From when the idea was mooted  till when it held its successful national convention last month, the PDP  and its supporters manufactured, sold and cultivated the notion that  the APC will never come to be. To their chagrin, at every turn, the APC  walks away intact, growing from strength to strength and buoyed each day  by the support of Nigerians who are frustrated that in sixteen years of  misrule. </div><div></div><div>PDP has ruled Nigeria for 16 awful years and what better report of its  stewardship than that  life has practically screeched to a halt as  nothing works in a country of estimated 160 million which <span class="q81g0lm7" id="q81g0lm7_7">international economic</span> experts project will have the largest number of poor people in 2015.  Yes, Nigeria is being projected to have more poor people than India,  with 1.1 billion people and China, with 1.3 billion people! These are  the three top nations habouring the largest number of poor people  globally. You can understand with China and India, with their over one  billion people, but with Nigeria? It is a shame! This was the PDP legacy  that informed the birth of APC.<br /><br />Despite the doomsday prophecies and wishes of the PDP and its supporters, APC was registered as a <span class="q81g0lm7" id="q81g0lm7_9">political Party</span> and it immediately became the biggest opposition party in Nigeria. Few  months after registration some governors moved from PDP to APC and  instantly took control of 16 states in Nigeria and PDP was left with 18  states. For a party that once controlled 33 states but now tentatively  holding to 18 states, it was a real and potent danger and it had to  fight back by every dirty means at its disposal. This is the straw that  broke the camel’s back. Anxiety, panic, confusion, fear gripped PDP  right, left and centre. Not knowing how to deal with the enormity of the  challenges facing them and the imminent danger of collapse of the party  the leaders of the party including President Dr Goodluck Jonathan have  resorted to blackmail, intimidation, subterfuge, harassment, religious  politics, propaganda, bribery, influence peddling, and <span class="q81g0lm7" id="q81g0lm7_6">name calling</span>.<br /><br />First they said APC is made up of failed politicians and rascals. When  APC began to wax stronger despite their gambit and lies, they started  insinuating that APC has links with Boko Haram. Infact, they claim that  APC is funding Boko Haram. Soon and in their own very eyes  they tagged  APC a Boko Haram party. Now this is the greatest danger. If you tell a  lies a thousand times, some ignorant people will begin to see it as  true. Now these lies must stop.<br /><br />Let us start with the history of kidnapping and hostage taking in Nigeria. It all started when PDP governors in the Niger <span class="q81g0lm7" id="q81g0lm7_16">Delta</span> started arming gangs and cults to steal elections for them. After  election, these gangsters used the guns to start a ferocious act of  insecurity that blossomed into general insecurity that has crippled many  states in Nigeria today.<br /><br />On Boko Haram, let us recall it started during the tenure of late  President Yar’Adua when PDP boasted of having an unlimited control of  the country with a vast number of states in its control. Then, there was  no APC and PDP rumbled from one end of the country to another. As this  became worrisome, Yar’Adua rounded up the leader of Boko Haram and  killed and this led to a declaration of war by Boko Haram. Because  Jonathan and his handlers believed that it can con sympathy from  Nigerians, it did nothing and allowed Boko Haram to overwhelm the  country’s <span class="q81g0lm7" id="q81g0lm7_19">security</span>. Meanwhile, Nigerians still recall the advent of bombing in Abuja when an <span class="q81g0lm7" id="q81g0lm7_11">Independence Day</span> bomb incidence shattered the peace of Abuja. Jonathan and his handlers,  playing desperately to its deadly politics of benefitting from the woes  of the country, blamed it on its opponents, even when MEND claimed  responsibility for the incidence. Later, Raymond Dokpesi was alleged to  be behind the bombing. Today, Nigerians still ask what happened to the  investigation on  that incidence or had it been buried in PDP’s deep  labyrinth of conspiracy?<br /><br />The late General Azazi, <span class="q81g0lm7" id="q81g0lm7_20">former</span> <div data-cid="2378" data-kw="national security" style="border-bottom: 1px solid; color: #f35b00; cursor: pointer; display: inline-block; height: auto; padding-bottom: 1px; text-decoration: underline; width: auto;">National Security</div>Adviser to the president told Nigerians at the beginning that Boko  Haram was a product of PDP’s infighting and struggle for political <span class="q81g0lm7" id="q81g0lm7_18">office</span> and supremacy. As the National Security Adviser and a kinsman of  Jonathan and a retired five star General, the late Azazi knew his  terrain and domain very well. He understood the game very well, he was  familiar with this kind of movements and struggles, he understood the <span class="q81g0lm7" id="q81g0lm7_15">job</span> and he had information from above and below. So when he said that Boko Haram was a product of PDP’s craze for <span class="q81g0lm7" id="q81g0lm7_14">power</span> was that accusation investigated? Did anybody take him serious? Was he  given the chance to prove his case? No they did not. They sacked him as  NSA and the man eventually died under questionable circumstances in a <span class="q81g0lm7" id="q81g0lm7_3">plane crash</span>.<br /><br />Have we recalled that Senator Ali Ndume a PDP senator was once quizzed  for Boko Haram allegations and he swore to an affidavit that Vice  President Namadi Sambo approved his contacts with Boko Haram. Pray what  had a government that steals trillions of Naira every year on the guise  of security votes but which believes that its opponents must carry its  cross, do about this grievous allegation?<br /><br />We have seen this type of game plan before in the days of the locust.  The late General Abacha threw bombs in Lagos and went ahead to arrest  the Yoruba leaders and accused them of being responsible for the crime.  When Alhaja Kudirat Abiola was murdered also by Abacha agents of death,  the same Abacha gave orders that some prominent Yoruba leaders for the  heinous crime. This is the way of dictators all over the world.<br /><br />The man died and so were his findings and reports. APC offered solutions  but was ignored by the powers that be. Today a party that went through  many troubles to be where it is now is being given a bad name in order  to destroy it, and some Nigerians are ignorantly buying the idea hook,  line and sinker. Initially when Boko Haram was targeting Churches, motor  parks, government buildings many Nigerians including this writer  thought the war was targeted on Southerners and Christians only until  they started attacking everybody including their own brothers, sisters,  school girls and even some traditional leaders in the North. Right now,  nobody is safe anywhere in Nigeria. Everybody, every institution, every  market, every school is now a target.<br /><br />Boko Haram is no longer a threat to Northern Nigeria only; it is now a  threat to Nigeria. It is no longer a Northern affair because nobody is  safe. To me I think it is now a global scourge. They came to Mali and  France forced them out. They are in Kenya, they are in Cameroun etc. But  what do the government ran by PDP do in the face of this ruinous threat  on the entire Nigerian polity? It signs off trillions of Naira as  security votes every year and makes a song of its opponents sponsoring  Boko Haram. As a party in government, the onus falls on PDP to prove its  case but it is not doing so because they want these to remain fables to  be used to shore up their politics after failing woefully in  governance. For a party that met 40 million <span class="q81g0lm7" id="q81g0lm7_13">poor</span> people in 1999 and has grown the figure to 130 million (World Bank  statistics), it is right and fitting to use Boko Haram to tarnish its  opponents and rip off Nigerians with continued stay in power.<br /><br />I think APC leaders must roll-up their sleeves as it did when the legacy  parties came together. They must fight as they did before the  registration of the great party. <span class="q81g0lm7" id="q81g0lm7_17">Yes</span> APC must fight back and those who know must also stand up to be  counted. If APC and well informed Nigerians allow PDP to continue to  sell this bad news to Nigeria and the world, then the real danger is  here.<br /><br />The world must know also that President Goodluck Jonathan has not helped  matters in the fight against Boko Haram. He spends more time in the  Churches these days as if he is a president of Christians in Nigeria  only. This is dangerous. He is the president of 150million Nigerians. He  and his party have polarized Nigerians so badly along religious lines  and this continues to conflate the Boko Haram crisis. Mr. President  controls the Army, Navy, Police, Airforce, DMI, SSS, NDLEA, Mobile  Police etc. but APC control nothing. Now if APC funds Boko Haram and the  President and PDP that control all the machineries of law enforcement  agencies cannot discover Boko Haram base and who are behind them, then  something is wrong. </div><div></div><div>APC has invited the world to investigate Boko Haram activities and their  sponsors in order to put to rest these allegations. We have called on  credible international investigative agencies to dip deep and unravel  the real sponsors of Boko Haram for the world to see that the suspicion  that PDP is responsible for the origin of Boko Haram and tactically  contributes to its present intractable nature because in its thinking,  it will help its very bad electoral standing and it will give them  enough grounds to continue pillaging security funds as we have seen in  Nigeria today. </div><div></div><div>This is the way to go, and Nigerians need this investigation to unravel  the misery of Boko Haram. We expect the PDP to take up this challenge  and join us to call for credible international probe of Boko Haram  origin, sponsors and sources of strength.</div></div>

Shekau also said his  loyalists carried out twin car bombings in May in Jos and a June attack  at a college in Kano.<br /><br /><b>Shekau claims Abuja, Lagos bomb attacks</b><br />“We were the ones who detonated the bomb in filthy Abuja,” Shekau said  in the video referring to a June 25 attack on a popular shopping centre  in Abuja that killed at least 22 people. Later <span class="q81g0lm7" id="q81g0lm7_11">that day</span> a huge explosion also rocked the Apapa, which the authorities blamed on a cooking <span class="q81g0lm7" id="q81g0lm7_6">gas explosion</span>, with no casualties.<br /><br />“A bomb went off in Lagos. I ordered the bomber who went and detonated  it,” Shekau says in the video, which shows him flanked by at least 10  gunmen in front of two armoured personnel carriers and two pick-up  trucks. “You said it was a <span class="q81g0lm7" id="q81g0lm7_4">fire incident</span>,” he added. “Well, if you hide it from people you can’t hide it from Allah.”<br /><br />Shekau, in boasting about the Lagos attack, incorrectly identifies the  Governor of Lagos State, taunting Adams Oshiomhole, who is in fact the  Governor of Edo State. Lagos State is governed by Babatunde Fashola.<br /><br />Declared a global terrorist by the <span class="q81g0lm7" id="q81g0lm7_3">United States</span> and sanctioned by the UN Security Council, Shekau is thought to have  previously claimed credit for attacks that he did not directly order.<br /><br />Near the beginning of the video, Shekau calls several of the world’s most prominent Islamist extremists his “brethren.”<br /><br />“May Allah protect you,” he said, listing IS chief, Abu Bakr  al-Baghdadi, Al-Qaeda head Ayman al-Zawahiri, Taliban leader Mullah Omar  and several others.<br /><br /><span class="q81g0lm7" id="q81g0lm7_21">Speaking</span> in English, Shekau recited the BringBackOurGirls slogan used by activists to press for the release of schoolgirls.<br /><br />“BringBackOurGirls… bring back our army,” he says, in what may be  reference to a previous offer that the hostages could be freed if the  Goodluck Jonathan governor releases Boko Haram fighters held in custody.</div></div>
